Dans cet article, les auteurs cherchent à montrer comment l'utilisation du Plan d'intervention individualisé interdisciplinaire (P3I) leur a permis, dans le cadre de la prise en charge des patients cérébro-lésés de l'Hôpital de jour de Coubert, de mener à bien des actions de réadaptation. Plus qu'un support écrit, il apporte de la cohésion à l'équipe et permet le dialogue avec la famille.
L'étude de cas montre le glissement des actes de rééducation vers des actes de réadaptation pour répondre au projet de vie du patient et maintenir le lien social.
L'observation statistique leur a permis un travail d'autocritique de leur méthodologie et, ainsi, de faire évoluer leur support écrit.

In this article, the authors try to show how the use of the individualized interdisciplinary intervention plan (P3I) allowed them, within the framework of care of the brain injuried patients of the day Hospital of Coubert, to carry out many actions of readjustment. More than one writting support, it brings cohesion to the professional team and allows the dialogue with the familly.
The study of case shows the drop of the reeducation acts towards readjustment acts to answer the project of the patient's life and to maintain the social connection.
The statistical observation allowed them a work of self-cristicism of their methodology, and thus to make advance their writting support.
